Evaluation forms built in EV+ can be added to existing credentials through the use of an Evaluation topic type.
The Evaluation topic type has several settings that let administrators control how completed evaluations count toward the topic.
Evaluation topic types work much like Hour and Unit based topics. The designated requirements of ALL topic types must be met in order for the credential to be completed.

Evaluation Topic Type
To add a topic in which to add EV+ evaluation forms, click Add Top and select "evaluations" in the requirements section dropdown menu. EV+ evaluation forms can ONLY be added to this topic type.
The number of evaluations entered refers to the number of evaluations that must be completed or passed within the user's credential Start and Expiration Dates in order to complete the requirement.
The first setting within the topic allows you to control whether the evaluation form must be passed or simply completed in order for your user to receive credit.
The second setting should be familiar. Like with courses/activities within Hour or Unit based topics, you as the administrator can determine if the evaluation form can be completed only once, or more than once, for credit toward the topic requirement
In the example above, there are 18 different skill sheets (evaluation forms) and each user must pass each one. Because the second setting is set to Once, the users must pass all 18 EV+ forms before completing the requirement.
Adding Evaluation Forms
Only EV+ evaluation forms can be added to Evaluation based topic types. To add EV+ forms, click the green plus icon. This opens a modal containing all EV+ forms with a Published status. Forms already added to the topic will not appear.
Once added to the topic, users with completed or passed (depending on the topic setting) evaluations using these forms, will count toward the topic requirement.
What Constitutes a "Pass"?
For an evaluation to be designated as passed, the form itself must have a pass/fail question type set to "Use as form pass/fail status".
If an evaluation is completed for a user using a form WITHOUT this specific question type and setting, the evaluation will always show as completed, and not as passed.
Similarly, if an evaluation is completed for a user using a form with the proper final pass/fail question, but the user fails the evaluation, the evaluation will show as completed, and not as passed.
Adding a Final Pass/Fail Question Type to an EV+ Form
To add a final Pass/Fail Question Type to an EV+ form, go to the Forms feature in EV+. Within your Unpublished form, click Add a Question and select "Pass/Fail" under type. Make sure to turn ON the "Use as form pass/fail status" setting.
