Create the Form
- Click Forms in the Administrator Pages left navigation
- Click the green Add Form button in the upper right
- The form build page will open and allow you to fill-in your form information and settings:

Name | Name of the form |
Group Names |
Group name (e.g. For Evaluators) that you want the form to display in the forms list when users click on New to initiate an evaluation. |
Order Within Groups |
Enter a number here to specify the order that you want the form to display in the in the forms list under drop-down menu when users click on New to initiate an evaluation. |
Description |
This information displays in System Administrators' view only so you may use any description that can help differentiate the form and its content from other forms. |
Type | Type of the form (Normal by default, Read-only, Computed.) |
Include Elapsed-Time Recording |
Setting to enable tracking elapsed time if the form requires capturing time duration. |
Observation Form |
Setting to include form in "Observation by Evaluator" & "Observation by Evaluee" Reports. |
Allow Privacy Control |
Setting to enable privacy control. Privacy control is useful when evaluator wants no one else to see the form while editing (Sharing OFF, Privacy ON), no other evaluators can see the form or when evaluators wants form to just be seen by evaluee (Sharing ON, Privacy ON) |
Allow Form Name Editing |
Setting to allow evaluators to edit the name of the form while performing the evaluation. (Frequently used for Notes.) |
Allow 'Pass All' option for Pass/Fail Questions |
Setting to enable the Pass All function to be used by evaluators when performing an evaluation. See Using the Pass All Setting help article for more information. |
Prevent use for self evaluation |
Setting to restrict users from using the form to perform a self evaluation. When this setting is restricted: Web: If the user is within their own personnel file and clicks New to initiate an evaluation, the form will not appear in the available forms list. Mobile: If the form is selected, the user’s own name will not appear in their available user list. |
Sharing Behavior |
Dropdown menu to define sharing behavior. It is set to "Author can share" by default. You may change it to, "Any evaluator can share", which will allow any evaluator to share the form, and upon author signing the form, the "Share" check box in the dialogue window will be unchecked by default. You may also change it to, "Sharing disabled", which will make it so that the form can never be shared. You have the option of checking the "Required for form completion" box. This requires that the form be shared with the evaluee in order to reach a "Complete" status. For more information about sharing, check out Sharing an Evaluation. |
Signature Behavior |
Drop-down menu to define signature behavior. It is set to "Signature locks signer from further editing" by default. This means an individual signature locks only signer's questions, but leaves the rest of the questions open for editing. You may change it to "First signature locks all questions", which locks all the questions in the form as soon as the first signature is completed OR you may change it to "Signature locks all questions prior to signature", which locks all questions above the signature in the form. |
Status | Status of the form (Unpublished by default, Published, Superseded, Archived, Publish-restricted) |
- Click Save and the form will be created and added to the list of your forms in the Unpublished status.
Add Questions
In the Questions section located beneath the form settings, click the Add a Question button.
Select the question Type from the dropdown menu.

Batch Edit Questions
There are some question settings that are universal. To change these universal settings for multiple questions at once, first use the checkboxes to select the questions you'd like to edit or delete.
Then select the Batch menu to edit or delete the questions. Note questions cannot be deleted and only certain edit options are available if the form is NOT in an Unpublished state.
