Question types are the different options for the questions inside your forms. Most all of our question types you can customize for your district. We have several different question types listed below. Before adding question be sure to learn about Question Subtypes as well.

Heading: This is used for form titles, School names, standards, indicators etc. |

Instructions: Can be used for instructions anywhere in the form. Contains and header and a body to put text in. | ![]() |
Text Entry: Includes header and instruction area and gives a comment box for the user to type in | ![]() |
Rating Scale: A horizontal rating scale with a header and description an option of pre made or customizable rating scale titles and values. | ![]() |
Choose One: allows only ONE selection for an answer. | ![]() |
Choose Any: allows MULTIPLE selections for an answer. | ![]() |
Date, Time, or Date Time: allows entering a particular date, time, or both. | ![]() |
Signature (locking and non-locking):Leaves an area for a user to sign the form to signify they have read and approved the form. | ![]() |
Script: Allows user to take quick notes throughout your observation, that are timestamped as it is happening. | ![]() |
Pass/Fail: Allows you add simple pass/fail questions to forms. Check the "Use as form pass/fail status" option when adding the question to the form to allow the question to count as the overall pass/fail for the evaluation completion. | ![]() |
